Fill in a survey about your experience

All patients are invited to fill in a survey after their experience with us. This is called the Friends and Family Test. It asks all patients the same question: “Overall, how was your experience of our service?”

You can fill in the survey in different ways:

• By scanning a QR code on a postcard
• By clicking one the link further down this section.
• By using an app on our Trust iPads
• By filling in a paper survey

We want to know what we are doing well and what we aren’t. It will help us understand what we need to do to change things for the better.
